School advice for a language impaired 7 year old?

I have a son with speech/language delay/disorder (At 7 he has an overall speech/language skills of a 51/2 year old, primer reading skills and grade level math skills). last year, he got a medical dx of a significant Attention disorder(inattentive ADD). Once you get him to focus on the task, he can do it pretty well, but he needs constant adult re-direction. He is a sloppy daydreamer who loses track of his task at the drop of a pen.

Other than this he is a bright, cheerful and friendly 7 year old. He has been mislabeled with a “Autism’ label by the school. I was kind of tricked into accepting the label 2 years back since he was too young for a medical diagnosis of ADD. Even though he is partially mainstreamed, I see that he is hurting socially and academically. His unique needs are not met completely but he is being used in the autism room as a peer-model for the behaviorally challenged. With his poor attention and anxiety he can’t learn a thing in the Autism room. The school refuses to change his label and classroom setting, since, he still needs para support no matter where he is placed.

1. I would like any advice regarding the public school system and how to avoid/navigate from such dead ends.

2. Do you think he can benefit from a montessori education until I figure out a way to find the best classroom fit for him? I was lucky enough to find a school which is willing to provide him more structure.
Thanks both of you for the replies. To knifeman, My son is in the public school in a self-contained special ed classroom half of the time and in mainstream for reading and math. My problem is, his unique needs are not being met in either rooms. That’s why I asked about Montessori. he has speech and language delay due to prematurity and seizures. In SLP terminolgy he has “scattered expressive-receptive speech/language delay”. ADHD is a recent dx which is linked with seizures.

To Kirsten, I accepted the label 2 years back because my son needed an IEP. Now I am trapped forever because my son is not progressing on the school’s timeline and the reason why he isn’t progressing, namely ADHD, is not being addressed by the Autism staff or the mainstream staff. He is ignored based on technicalities.

I need an SLP advice from you.

If the underlying reason for speech-Language delay is not specific, do you think a combination therapy approach would be helpful. I mean, Do you think seeing two speech therapists at the same time is a good idea?

One therapist is from the hospital who will give intensive speech therapy with goals like –>XYZ will undersrtand directions that contain ‘next to’ and ‘in between’ 90% of the time.

The other speech teacher will have a holistic approach. She’ll help with social language development, classroom adjustments, tell us when to use augmentations like picture remainders etc

First, I would get second opinion evaluations from outside agencies (talk to you doctor about getting referrals to the correct agencies). Include an eval from a speech pathologist–to get the langauge delay on record, and a psychologist who will address the ADD and Autism labels in his eval. You may need to have the autism label revoked to get him out of that classroom. You can take these reports to you school district, and see if it helps amend the IEP. You may also feel the need to contact an educational lawyer, someone who specializes in getting school districts to understand the legal ramifications for not getting your son’s needs met in his current classroom. I am hearing impaired and I got an urgent letter from Sorenson Video Relay service… And its saying that Bush is deciding to cut – off VRS and make all of us Hearing Impairs go back to TTY/TTD where we have to suffer from dealing with LOOONG delays of GA and SK and so on. You are probably puzzled about Video Relay Service, are you? go to www.sorensonvrs.com and you will see what it is. I’ll give you a description of it: Its like a webcam, and when I make a phone call to a hearing individual, an interpreter comes up and she/he calls the number I provide. And we wait till a person picks up and answers. Then we use sign language to communicate. its FAST and EASY to understand anyone w/o having to use words and it so much fast for us. If you really do want to help us, all you have to do is type in a “signature” and I will copy this after the end of this week and mail it to my congressman/woman. Here’s mine:

We deaf people are trying to keep this VP going w/o having to go back to TTD and TTYs. So if someone supports our needs, they and use a “signature” to tell our government that we want to keep VP going for as long as it lives. Since we do really need it. Its Bush that is deciding that cause HE is paying for it. Its really hard to explain… He pays the interpreters to interprete for us, he pays the bill that we call to others to communicate. Its free for us. If we can get a lot of signatures, we can continue using Video Relay Service. and not TTYs.

In addition, our congress does all the work to debate in the house of reps and demos to pass another bill to have the president sign it as acception or veto it. Why is Bush trying to cut medicaid and medicare off? Surely he’s trying to do that to our ADA provided services, too. Not everyone can survive.

(b) autism apparently inflexible adherence to specific, nonfunctional routines or rituals
(c) stereotyped and repetitive motor mannerisms (e.g., hand or finger flapping or twisting, or complex whole-body movements or with an object)
(d) autism persistent of liking an object, movements with sound.

(B) Delays or abnormal functioning in at least one of the following areas,
with onset around 2 and half of age to 3 and half years old:
(1) social interaction,
(2) language as used in social communication, or
(3) symbolic or imaginative play (playing or talking some form of outer space words or cartoon’s words or play).

Symptoms of Asperser’s include: impaired ability to utilize social cues such as body language, irony, or other subtext of communication; restricted eye contact and socialization; limited range of encyclopedic interests; preservative, odd behaviors; didactic, verbose, monotone, droning voice; concrete thinking; over-sensitivity to certain stimuli; and unusual movements.

All these are the symptom of autism and we have the treatment for all these problem. You have to spend enough time for the intensive treatment in order to get good result, it varies from 1 month to 6 months or more for intensive treatment (remedies, remedy).

The True Story From Korea
Joe is 8 years old an Autistic Kid from Korea. Even though Korea recognized acupuncture and Joe’s grand father is the top acupuncture physician there, he can’t help. Others specialist can not do much for remedy.

Complain :
Doctor has been authenticating that he has autistic since 1 year old. His families already tried all kinds of treatment and therapies to get him out from autism. Easily distracted.
Forgetful in the course of daily activities.

Hyperactivity

A child must have exhibited at least three of the following symptoms for at least six months to an extent that is unusual for their age and level of intelligence.
Runs around or excessively climbs over things
Unduly noisy in playing, or has difficulty in engaging in quiet leisure activities.
Leaves seat in classroom or in other situations where remaining seated is expected.
Fidgets with hands or feet or squirms on seat.
Impulsivity

At least one of the following symptoms must have persisted at least for six months to an extent that is unusual for their age and level of intelligence.
Blurts out answers before the questions have been completed. Fails to wait in lines or await turns in games or group situations. Interrupts or intrudes on others, e.g. jump into others conversations or games.
Talks excessively without appropriate response to social restraint.

Pervasiveness of attention difficulties and hyperactivity

For a diagnosis or description of ADHD a child would be expected to show the above difficulties in more than one setting, e.g. at school and at home.

Problems are not shown ‘at home’ but are very evident when a child goes to a hospital department. This can happen when parents do not realize that their child’s behavior is out of the normal range (perhaps because they have no other children, or they have other children who behave similarly). It may also be because the problems are mild, or because the family has handled the attention lack at home in such a way that it is not evident there is a major problem, or because the child is very young. In those cases it is quite reasonable for parents not to consider that their child has an attention deficit problem.
These conditions has a good chance to be treated, it will take time and effort to do it. The Tole’s way of remedies for them is not easy and tedious, the good part is it works with many of them.

Fails to follow through instructions or to finish homework (not because of confrontational behavior or failure to understand instructions).
Disorganized about tasks and activities.
Avoids tasks like homework that require sustained mental effort.
Loses things necessary for certain tasks or activities, such as pencils, books or toys.
Easily distracted.
Forgetful in the course of daily activities.

autism- Hyperactivity
A child must have exhibited at least three of the following symptoms for at least three months to an extent that is unusual for their age and level of intelligence.
Runs around or excessively climbs over things. (In adolescents or adults only feelings of restlessness may occur.)
Unduly noisy in playing, or has difficulty in engaging in quiet leisure activities.
Leaves seat in classroom or in other situations where remaining seated is expected.
Fidgets with hands or feet or squirms on seat.

autism ADHD Impulsivity
At least one of the following symptoms must have persisted at least for three months to an extent that is unusual for their age and level of intelligence:
Blurts out answers before the questions have been completed.
Fails to wait in lines or await turns in games or group situations.
Interrupts or intrudes on others, likes to jump into others conversations or games.
Talks excessively without appropriate response to social restraint.

ADHD or Others causes
Epileptic seizures can cause a child to become drowsy, impairing their attention. Epilepsy can also cause unusual behavior and lead to abnormal perceptions.
Hearing problems such as deafness or glue ear can make it hard for a child to follow instructions and make them appear inattentive.
Reading problems, making it hard to complete tasks or follow instructions.
Obsessive compulsive disorder leads to people following strange rituals that preoccupy their thoughts and distract their attention.
Tourette’s syndrome involves repetitive, involuntary jerking movements of the body and sudden outbursts of noise or swearing.
Autism and Asperger’s syndrome often lead to difficulties in understanding and using language.
Prolonged periods of insufficient sleep, causing poor concentration.

Some children may be very active or be easily distracted or have difficulty concentrating. If these behaviours are relatively mild, they should not be considered a disorder.

Others difficulties that occurs alongside Autism ADHD
ADHD often occurs alongside other difficulties and is not the sole cause of problem behavior. Children may exhibit temper tantrums, sleep disorders, and be clumsy. Other behavioral problems that occur with ADHD include:
confrontational defiant behavior, which occurs in most of these kids. The child loses their temper, argues and refuses to comply with adults and deliberately annoys others.

conduct disorders occur in at least 25 per cent of children. The child may be destructive or show deceitful behavior such as lying, breaking rules and stealing.

specific learning difficulties, including dyslexia, occur in 25-30 per cent of children.

severe clinical depression occurs in about 30 per cent of children.

anxiety disorders occur in 30 per cent of children.

What causes ADHD?
Biological
Temperamental, as this contributes to their attitude and personality.
Twins suggest a genetic link to ADHD. In 80-90 per cent of identical twins where one has ADHD so does the other. Recent research also suggests there is a greater chance of inheriting the condition from male relatives such as grandfathers and uncles.

Brain injuries due to birth trauma or pre-birth problems. The brain structures believed to be linked to the development of ADHD are vulnerable to hypoxic damage during birth. The damage is caused by inadequate oxygen (QI flow) reaching parts of the brain while blood flow is reduced.

Environmental conditions:

Family stress.
Educational difficulties.

ADHD diagnosis
Observations to the assessment of a child with possible ADHD. There is no single diagnostic test for ADHD so different sorts of information needs to be gathered, such as the following:
History of symptoms The precise nature of the difficulties, when they were first noticed, in what situations they occur, factors that exacerbate or relieve them.
Medical history
Risk factors that could predispose the child to ADHD include difficulties and risks in pregnancy and during birth, for example if the mother was in poor health, very young or drank alcohol or smoked or had an extended or complicated labor.

Several medical conditions are known to be associated with ADHD. These include fragile-X syndrome, fetal alcohol syndrome, G6PD deficiency, phenylketonuria and generalized resistance to thyroid hormone.

Accidents, operations and chronic medical conditions such as epilepsy, asthma and heart, liver and kidney disorders all need to be taken in to account. Also of possible relevance is any medication the child is taking, as well as any adverse reactions they have had to medication in the past.

Past psychiatric history:
Enquiring about any mental health problems the child has had can help rule out depression or anxiety being behind the symptoms.

Educational history:
This means the level of their ability and what specific difficulties they have, how they function within their peer group and get on with teachers, and any behavior difficulties such as suspensions or exclusions.

Family history:
The mental and physical health of the child’s parents and other family members can be relevant, particularly regarding the incidence of ADHD or depression. Less then 1% ahs the family history but this is one of the research data base.

Social assessment:
The family’s social circumstances, such as housing, poverty, and social support may all have an impact on the child’s behavior. Less then 4 % has this problem but it should be taken into consideration.

I think that this like many other things that have come and gone in the past years is unfortunatly not a cure. People are always claiming that a diet or something is going to cure autism. I work with children with autism and I’ve never heard about this working. I know 5 years ago when they thought vaccines may be causing autism it was all over the news. I think if it did it would be on the news and all over the place – because a cure for autism would be a big deal. This may be a good treatment for some children but I don’t think cure is the right word. (This of course is just my personal opinion and it would never hurt to try a new treatment)

Howcan I get back together with my love?

OK, here is your mission, if you choose to accept it… My girlfriend of 3 years and 8 months told me last week that she had to make some important decisions in her life, and needed her independance. We were both very upset about it. After a long conversation, we decided to take a break for a while, and I put everything in her control; she doesn’t want to make the wrong decision. The problem is that this is killing me, more than I ever thought it would. I call her every other day, and she will talk… but not about us and will get mad if I mention it. I really want this to work out; I’ve never wanted anything more. What should I do to show her this? I think she just wants to feel independance, and is not seeking another boyfriend at this time… now what?

Well friend, all you can do for now is respect her wishes. I know it is hard to do, but sometimes after being with someone for so long, we loose sight of being an individual. Your girl might just be going through a phase, that she sorta lost herself along the way. Give her her alone time, and stop calling her so often. She needs time to miss you, and if you are always there it will seem as if nothing has changed. If you truly love her, summon up the courage to let her be. Let your girl call you, and allow her to bring up the issue of getting back together. If she gets angry because you are pushing her, then by all means do not discuss it, follow her lead on this one.
